In order to fill your Canada Online Pharmacy prescription, we
require certain information from you.

This is a short 2-step process:

1. Fill out the Patient Questionnaire and Release Form
You will be required to complete a patient questionnaire and to sign a release form. The patient questionnaire and prescription are reviewed by a licensed online Canadian physician. This enables us to safely and efficiently fill your canadian prescription drugs. Any information you provide will be kept strictly confidential.

2. Send us the information.
Simply mail the forms back to us, or fax them back to us toll-free at 1-866-972-8379. Please remember to include your valid prescription with the questionnaire and release form.

Approval and Shipping
We will contact you to confirm all orders that we receive by fax or mail. Please allow 3 to 4 business days for us to process your order along with 5 to 7 business days for shipping.

At this time, the United States Food and Drug Administration (FDA) limits the quantity of medication we can send you to a 3 months supply. If your prescription allows refills, you can order these refills from us every 3 months. You are not required to fill out another questionnaire or release form to order a refill. Once the refills are exhausted, you will be required to submit a new prescription. If there have been no changes to your prescription you will not be required to fill out another questionnaire or release form.

We DO NOT ship controlled substances such as amphetamines, benzodiazepines (e.g. Valium), or narcotics such as codeine and morphine.

Under pharmaceutical law, prescription drugs are not returnable. All sales are final.
